1. Personal information to be handled

Personal Information that we handle on e-SMBG system (hereby referred to as this system), shall include the following customer information

(1) Personally identifiable information that we collect when customers register to e-SMBG service from this system, including Name (including Nickname), E-mail Address, Date of birth, Sex, Prefectures of your address, etc.

(2) Personally identifiable information that we collect when customers apply for a tester, including Name, Address, Phone number, E-mail address, etc.

(3) Personally identifiable information that we collect when customers reply to the questionnaire, including Name, Address, Phone number, E-mail address, Date of birth, Hobby, Physical information, Lifestyle, etc.

2. Intended use of personal data

Concerning 1(1), we shall use the personal information only for providing e-SMBG service that the customer registered for

As for 1(2)(3), we shall use the personal information only for sending information about trial products, sending the actual trial products, sending information about new products , sending questionnaires, and making a statistical analysis/ interpretation of questionnaire results.

3. Provision of personal information to third parties

(1) We shall not provide personal information to third parties except in the following cases.

Ⅰ．If required by the Personal Information Protection Act in Japan or other applicable laws and regulations.

Ⅱ．If the customer has agreed.

(2) If our company outsources a part of this system to a third party, it would be deemed that prior consent has been obtained from the customer. However, in this case,

we will require them to keep the confidentiality of personal information of customers and to prohibit re-provision to other third parties. We will also implement appropriate management of personal information of customers.

4. Correction or deletion of personal information

The customer is able to request us to correct, discontinue using or delete personal information, and we will respond by the registration screen, email or phone.

5. Our internal organization and efforts

For personal information that the customer provided, we shall have an assigned personal information supervisory manager and establish a personal information handling system in accordance with our regulations to ensure that all employees abide by regulations

6. About improvement of our handling system.

We shall ensure our personal information handling system conforms to rules and regulations under the laws of Japan and review it for continual improvement.
